It was great to meet you in Copenhagen a few weeks ago. You really did move a lot in my mind.As I told you I do make quite some full cases. I prep them in one session. My colleque Sören has now changed the articulators in our lab. We now have the same features as if we used the Accu-Liner.
We are very decisive, and we do see and understand your ideas about the HIP plane. It really make sense. So from now on we will work according to the principles we learned at your course with all "more than minor" cases.
The first full case made like this I had a few days ago for trying in before the final porcelain firing. And Chris, I guess this was the first full case I ever made where I didn't have to make any corrections. Before I always had to change something, maybe the symmetry, maybe the level of the smiline, maybe the length of the incisors. But this time: nothing.
Well it may be a coincidence. We will see. But it all make sense according to your philosophy.
By the way, one week ago a patient came in. She urgently asked for help because she had huge TMJ problems, and she was very unhappy with the appearance of her teeth, even after she had a lot of restoring. Every morning she woke up with ear pain and muscle pain and it more or less continued during the day. She had crowns and bridges for more than 20.000 CHF( I think about 18.000 USD) all made within the last two years. They were ok, but they looked ugly. She had a asymmetrical midline and smile and the teeth on her right were much longer than the on the left side when she smiled.
Well Chris, I must admit that I never gamble about money, but deep inside I am a little bit of a gambler. So although this was my first serious TMJ case since I visited the course in CPH, I gambled (or maybe on the contrary this time I made no gambling at all ! ). I made a thorough examination, and concluded that she really had TMJ problems. No muscle pain. and I told her that I would remove her pain within hours, and that I later could give her a beautiful smile! So I made a impression and made a mandibular decompression device, just as you have described on your DVD and at the course. This within three hours. She came the same day at noon, and I applied the device. The morning after she called me and told me that a miracle had happened. She was pain free! I told her that I was very happy for that, and at the same time she said, that she urgently wanted her teeth restored cosmetically and functionally. Well Chris, without your course I may have solved her problems any way, but much less effective and in a much more time consuming way. But this woman now knows, that I know my dentistry!, and she wants more dentistry from me! So now I will go on during the next weeks and find her natural plane/level of occlusion, and after that we will make the restoration. Thanks Chris!

Thank you for sending me a copy of your DVD on progressive appliance therapy. It is excellent and you are a great teacher. I cannot argue with any part of your protocol. You explain your techniques concisely and logically. It makes sense. Your lesson is very important.
I would recommend your DVD to anyone getting started in TMD or seeking a better perspective on diagnosis and therapy. It should be mandatory teaching for all dental students. It would represent the best 2 hours of their four year dental school experience.

"Stop practicing Hope dentistry" is a term we all know very well; I have just watched a DVD entitled "Progressive Appliance Therapy for TMD & Full Mouth Restoration" by Dr. Chris Stevens, a presentation by "Prosceedure Productions" of Utah, who had asked my opinion of it. Whilst I am not a paid representative I do take an active interest in NM dentistry and know of Dr. Stevens the author and presenter. Certainly without a clear understanding of neuromuscular dentistry, comprehensive care MUST be considered "Hope Dentistry" at best.
The technical quality of the DVD is quite simply excellent. It is such a pleasant change to see this quality of film making. It leaves one the luxury of concentrating on the presentation at hand and its content and Proseedure Productions are to be commended in bringing this level of quality to the dental continuing education arena.
Dr. Chris Stevens requires little introduction as, an American Dentist working in Utah, he is well know on lecture circuits there and abroad teaching NMD toward prosthodontic or orthodontic stabilisation. The DVD title promises and delivers the use of a progressive approach to appliance therapy for both symptomatic TMD patients and those non symptomatic, wishing full mouth reconstruction. The video describes the use and protocol of a logical sequence of appliances. It must be emphasised though, that it is not an introductory video and requires of its viewers some basic NM tenants. Dr. Stevens also modifies several concepts that again require the viewer to be in a position to evaluate such changes. Whilst the dentist with experience can intellectually evaluate such a modification in protocol, no scientific basis is provided which may assist the NM neophyte. An example is the end of a flat plane splint to facilitate muscle balancing over a "NM demand appliance" .Certainly, the ability to have more "user friendly ($) tools in your NM armamentarium" is a good thing.
In my opinion this video is worth the purchase and viewing. It’s a shame that there are so few video productions of this calibre available, but one is to take heart at the availability of this one. You might not agree with everything you see on continuing education courses, but as was once said "I may not completely agree with you, but I will defend your right to say it with my life" or words to that effect. We need videos of this calibre on this subject and I think this video belongs in the library of any dentist who seeks to provide comprehensive care for their patients. We need more videos of this calibre and in this clinical area, buy the video and support the production of more in this same area.
